Patients who are prescribed Blincyto, Imlygic, Kyprolis, or Nplate can enroll in Amgen Nurse Ambassador by completing the enrollment form and faxing it to 88. Your Amgen Nurse Ambassador can refer you to nonprofit organizations that may provide you with community resources. Whatever type of insurance you have, even if you have none, your Amgen Nurse Ambassador can help you understand how your Amgen medicine may be covered, and refer you to programs that may be able to help you afford it. When patients enroll in Amgen Assist 360, Amgen Nurse Ambassador is a single point of contact who can help patients find the resources that are most important to them. Application & Enrollment Amgen Assist 360 In certain cases, patients who are underinsured or who have Medicare Part D coverage may qualify to receive support. Have no insurance, or their insurance plan excludes the Amgen medicine.Satisfy income eligibility requirements. Have lived in the United States or its territories for ≥6 months.To qualify for this program, patients must: Must not be a resident of a state where coupons or copay programs are not valid.Must not have government insurance (eg, Medicare, Medicaid).Must have private commercial health insurance coverage for the prescribed medication.Must be prescribed an Amgen drug covered by the program.To qualify for the Amgen FIRST STEP program, a patient: Referrals to Other Foundationsįor patients with government insurance, Medicare, or Medicaid, Amgen Assist 360 also helps by providing referrals to independent nonprofit foundations that can help with paying for medications. The Amgen Safety Net Foundation is a nonprofit assistance program supported by Amgen that provides qualifying uninsured or underinsured patients with Amgen drugs at no cost. This program has no income eligibility requirements. Maximum program benefits are drug-specific and range from $10,000 to $20,000 per patient per calendar year.For subsequent doses, Amgen pays the patient’s combined out-of-pocket cost in excess of $5 per dose up to the brand program maximum benefit and for Prolia out-of-pocket cost of more than $25 with a maximum benefit of $1500 annually.
